This Corestream Privacy Policy constitutes an integral part of the Corestream Terms of Service Agreement. As used herein, the terms “Information,” “Data,” and “Service” have the meanings ascribed to them in the Corestream Terms of Service Agreement. As used herein, the terms “we,” “our” and “Corestream” refer to Empower Benefits Inc. dba Corestream.
This Privacy Policy applies to the Information and Data and instructions that we collect from you on or through the Service or insert on your computer by way of the Service.
THE INFORMATION AND DATA WE COLLECT AND INSERT
In order to access and utilize the Service, you must be eligible to do so, and you must complete the Service’s registration process, following which you will possess a unique username. For example, we use Information provided by you to confirm that you are eligible to utilize the Service and to determine the particular insurance and other products, benefits, discounts and programs that are available to you. We also use Information and Data supplied by you to communicate your decisions and preferences to the employer or association with which you are affiliated (if applicable) and to the providers of the insurance and other products, benefits, discounts and programs that you have designated. We also use the Information and Data you provide to improve the design and performance of the Service and, when necessary, to contact you.
On infrequent occasions we may need to obtain personal financial information from you, such as a credit card number and related information. In all cases, such financial information is manually collected by telephone, is immediately used for the specified reason, and is not retained by us.
We do not share personally identifiable Information and Data you have provided with any person or entity other than the business entity or association by which you are employed or with which you are associated (if applicable), except when it is necessary to do so in order to fulfill a request you have made. For example, it may be necessary to share Information and Data you have provided with the supplier of a program or service in which you have chosen to enroll, with periodic payments to be made to the provider through payroll deductions.
If required by law, by legal process, or by a Court order, we will disclose your personally identifiable information. We will notify you of our intention to do so before actually doing so, if such can feasibly and reasonably be done.
If you post a comment on the Corestream website but fail accurately to identify yourself, and if Corestream receives a complaint from a third person reasonably contending that your comment contains one or more defamatory and/or damaging statements, we will disclose your identity to the complainant upon request. If you do not wish for this to be done, you should entirely refrain from posting comments on the Corestream website that could reasonably be regarded as defamatory and/or damaging.
SECURITY

HOW YOU CAN ACCESS OR CORRECT INFORMATION
You may access and change or modify Information you previously provided. You may do so by directing an email to us at info@corestream.com. To protect your privacy and security, Corestream will take reasonable steps to verify your identify before granting access or making changes or modifications to your Information. Unfortunately, we cannot ensure that such changes or modifications will be reflected in all iterations of the Information as previously stored.
AMENDMENTS
We may amend this Privacy Policy from time to time without notice to you. You should periodically review the current version of this Privacy Policy. By continuing to use the Service subsequent to our publishing an amended version of this Privacy Policy, you thereby acknowledge, agree and consent to such amendment.
